Plan Before You Play
Dummy goes down and you count your losers in a suit contract. Spades:1, Hearts:1, Diamonds:0, Clubs:1. You should make this contract.
The Play
Win the diamond with AD, lead out KC then AC, lead out 10H and duck west wins and returns a diamond win with QD and play 9H and cover with AH. Lead 9S up to KS. Ruff the spade return, ruff a low heart, play 8D to the KD and the last 2 tricks go to your opponents.

As responder don't jump to 3C which shows 16+ points 5 hearts and 4 clubs. This is one of those tricky hands where you have enough points to be in 3NT but not solid spade stoppers. You have to take all your winners without losing the lead.
