Partner opens 1D you show 16+ points and a 5 card heart suit by jumping and bidding 2H (remember you could have bid 1H). Partner supports your hearts and you sign off in game. If partner had extra values over a minimum hand they would have Ace asked - bidding 4NT. But they haven't, so nothing can be gained by south Ace asking. All Pass

Partner opens 1NT - with your points you want to be certain of reaching game so you make a jump bid of 3H promising a 5 card heart suit and game points between you. Partner will Ace ask if they have extra values above a minimum opening hand. Partner bids game in hearts. All Pass
Plan Before You Play
Dummy goes down and you count your losers . 1 in spades and 1 in hearts. You will make 12 tricks if you can capture the QH. With 9 cards in hearts and no bids by the opposition you should play for the drop you must lose to the AS
The Play
Lose to the AS. Win the KS return and draw trumps. With 9 trumps play for the QH to drop, so play AH then KH The rest of the tricks are yours.
Comment
In this deal six hearts makes despite the lack of controls in South's hand if the QH can be dropped by leading out AH then KH - also the opposition holdings in hearts must be split either 3-1 (with a heart queen singleton), or 2-2 (heart queen on either side). A second condition which makes this game contract into a slam contract is if the heart queen can be finessed because its in East's hand, this can be accomplished with a 1-3 or even a 0-4 heart split.
